More Than Half Of Americans Have Battled Obesity, Study Says”

Metabolically healthy obese people were 49 percent more likely to develop cardiovascular disease and nearly twice as likely to develop heart failure as normal-weight people without any metabolic abnormalities, the study found.

Compared with patients who were metabolically unhealthy and obese, those who were metabolically healthy and obese were more likely to be men, younger, socioeconomically deprived and current smokers.

"Although those "metabolically healthy" obese people may not have those risk factors we described - diabetes and high blood pressure and blood fats - being obese is already a metabolic abnormality", said senior study author Neil Thomas of the University of Birmingham in the UK.

After adjusting for confounders, among those with no metabolic abnormalities, compared with those of normal weight, patients who were underweight had an increased risk for peripheral vascular disease (HR = 1.49; 95% CI, 1.36-1.63) but patients who were obese (HR = 0.91; 95% CI, 0.86-0.96) and overweight (HR = 0.92; 95% CI, 0.88-0.96) had decreased risk. Obesity increases the risk of heart disease, diabetes, joint disorders and certain cancers.

"Some researchers have called for a shift in the public health focus away from markers of adiposity, such as BMI, and have suggested that health providers prescribing weight loss interventions may be misusing time and resources".

Patients were categorized by body phenotypes, including underweight (BMI 18.5 kg/m), normal weight (BMI 18-25 kg/m), overweight (BMI 25-30 kg/m) and obese (BMI 30 kg/m).

"Clinicians need to be aware that individuals who would otherwise be considered nonobese based on a normal BMI can have metabolic abnormalities and therefore also be at high risk for CVD events", Caleyachetty and colleagues wrote.

"An adult who is 5' 9" (175 cm) tall and weighs from 125 lbs (57 kg) to 168 pounds (76 kg) would have a healthy weight and a BMI of 18.5 to 24.9, according to the U.S. Centers for Disease Control and Prevention. Another 15 percent were metabolically healthy and obese.

A new study is pointing to the growing health crisis surrounding obesity in America.

Importantly, it also showed that "normal" weight individuals with one or more metabolic abnormalities had an increased risk of CHD, cerebrovascular disease, heart failure and PVD compared to normal weight individuals without metabolic abnormalities.

Stokes' team also noted that Americans who had lost the weight were still at an increased risk of chronic disease. One in 10 normal-weight individuals had at least one metabolic issue, Thomas' group observed, and their disease rates did exceed those of their peers free of diabetes, hypertension, and hyperlipidemia.

Note that the data robustly challenge the assertion that metabolically healthy obesity is a benign condition, and indicate that metabolically healthy obesity is a high-risk state for future cardiovascular disease events.

People should not base their understanding of their own health and fitness on BMI alone, said Jennifer Bea, a researcher at the University of Arizona in Tucson and author of an accompanying editorial. By 2013 to 2014, 22 percent of people who weren't obese had been at some point, the study found. "Our research suggests that this could result in the failure to identify metabolic abnormalities, such as diabetes, high blood pressure and high cholesterol, in many normal weight patients".
