US Navy says deadly McCain collision was preventable; relieves ship commander”

The commanding officer had exercised poor judgment, and the executive officer exhibited poor leadership of the ship's training programme, it said.

The Navy has relieved both the commanding officer and the executive officer of USS John S. McCain of their duties over leadership failures related to the fatal crash with a merchant vessel that killed 10 sailors under their command.

The guided-missile destroyer USS John S. The USS John S. McCain crash marked the fourth major incident involving Navy ships in the western Pacific over the past year, including the June 17 collision of the destroyer USS Fitzgerald and a Philippine-flagged container ship southwest of Yokosuka, Japan that left seven sailors dead.

Pacific Fleet commander US Admiral Scott Swift also relieved three-star Admiral Joseph Aucoin, commander of the US Navy's 7th Fleet of duty shortly after the incident occurred. Navy officials concluded that the damage sustained by the McCain could be repaired in Japan which will allow sailors assigned to the ship and their families to stay nearby and not relocate to a stateside fix site.
