Risk of breast cancer's return looms for 20 years

Breast cancer can resurface after remaining dormant for 15 years following successful treatment, a study has found.

The research, carried out by The Early Breast Cancer Trialists' Collaborative Group, collected data from more than 60,000 women who had been diagnosed with hormone sensitive breast cancer (usually called oestrogen receptor positive or ER+ breast cancer) between 1976 and 2011.

The current treatment for women with ER+ breast cancer is for them to take anti-oestrogen therapy for five years, but in some cases they take it for longer.

Treatments for breast cancer have improved in recent years, so the estimated risks of recurrence may be on the high side, said co-lead author Richard Gray, of the University of Oxford.

Women whose original tumours were large enough to have spread to four or more lymph nodes had a 40 percent risk of a cancer recurrence over the next 15 years.

Doctors have long known that five years of tamoxifen reduces the risk of recurrence by about a third in the five years after stopping treatment.

"To assess 20-year risks, we had to study women who received their breast cancer diagnosis many years ago", said Gray. Aromatase inhibitors, which work only in post-menopausal women, are even more effective than tamoxifen at reducing recurrence and death from breast cancer.

For women with small, low-grade cancers and no spread to the lymph nodes, the risk of cancer recurring was 10 percent in 15 years.

Side effects can include hot flashes, vaginal dryness, osteoporosis and joint pain.

The research is published in the New England Journal of Medicine.

"We know that treatments have improved since then so women who were diagnosed more recently may be reassured that recurrence rates will be somewhat lower for them". At the five year mark the women had no signs that their breast cancer had come back and treatment ended.

In this trial, the researchers will use a focused ultrasound to destroy part of the primary tumor and then patients will receive an immunotherapy drug to prompt their immune system to kill the cancer cells.

"As we look at extending endocrine therapy for 10 years, we wanted to determine whether there were certain subgroups of women whose risk of recurrence was so low they might not need to continue endocrine therapy after five years", Hayes says.
