A week after the disaster, there is still no answer as to what caused the crash.

Transport safety committee chairman Soerjanto Tjahjono said airspeed indicator malfunctions on the jet's last four flights, which were revealed by an analysis of the flight data recorder, were intertwined with the sensor issue. "NTSC wants to explore this", he said.

"When there was a problem, the pilot would write it down and the mechanic would do (a repair).Then the plane would be declared airworthy".

Boeing's advisory said the plane experienced "erroneous input from one of its [angle of attack] sensors".

It has not been determined whether those incorrect readings played any role in the October 29 crash.

The place crash was the first involving the new Boeing model and is the latest version of the United States aircraft company's best-selling twin-engined plane.

Lion Air Corporate Communications Strategic Danang Mandala Prihantoro delivered apology over the incident.

"The bulletin reinforces existing procedures which all Air Canada crews are now trained on", Fitzpatrick said.

AMC personnel of the airport had been questioned by the authorities over the accident, he added. When the sensors detect such an aerodynamic stall, they send a command to the computers to point the aircraft's nose down to regain flight speed.

Angry relatives of the victims demanded answers from the airline's owner at an emotional press conference.

"We are not ideal human beings", he said, sobbing.

The device was replaced in Bali after pilots reported a problem with airspeed reading, the Indonesian transportation safety regulator said Wednesday.

All were found to be airworthy although two required repairs for "minor" problems.

The company went on to say that the investigation into what happened with Lion Air flight 610 is ongoing and that it would "co-operate fully and provide technical assistance" as requested.

The accident has resurrected concerns about Indonesia's poor air safety record, which until recently saw its carriers facing years-long bans from entering European Union and U.S. airspace. The plane plummeted into the sea a day after having a critical sensor replaced.

The pilot on the crashed Lion Air flight had asked shortly after takeoff to return to the airport in Jakarta but lost contact with air traffic controllers afterward. There were no survivors.

Boeing has issued a warning to airlines using its 737 MAX planes after a sensor failure was identified as a potential cause of the crash of a passenger flight near Indonesia. However, in rare instances, accidents have been caused by what investigators call a "startle factor".

The FAA has warned pilots of this happening and said it would "take further appropriate actions depending on the results of the investigation".

Indonesia's Transportation Ministry said it would investigate how the Boeing passenger plane damaged its left wing on the runway.

Data from the black box of the Lion Air 737 Max that fell into the sea with 189 people onboard has confirmed there was an issue with the plane's airspeed indicator.
